To start the recruitment process to become a Correctional Officer, WDOC requires an Official State of Wyoming Application, Background Questionnaire, and additional documents to complete the background investigation. As you are preparing to send in your forms and required documents, please use the checklist below to ensure that you have included all necessary paperwork. Sending in your forms and documents in a timely manner greatly speeds the recruitment process. Please call if you have any questions or concerns.

Please Include all of the Following:

• State of Wyoming Application
• Pre-employment Background Questionnaire
• IRS Form 8821. Below are the directions on how to fill out the form.

1. Please complete line one with your full name, address, social security number and phone number.
2. The appointee is Raquel Christensen Background Investigator for Wyoming Department of Corrections.
3. Tax matters are for employment purposes only. Please fill out (a) box with employment, (b) box n/a, (c) box 2005 thru 2009 and (d) box n/a.
4. Please check box four.
5. Finish by signing the left hand side. 

• IRS Form 8821
• Copy of High School Diploma/GED and/or college transcripts
• Copy of Corrections Training Records (if applicable)
• Copy of Credit Report
• Copy of Current Drivers License
• Copy of Social Security Card, Passport, or Birth Certificate
• Copy of DD 214 (if applicable)
